Assumption of Risk and Waiver of Liability

Your participation and/or attendance at any event, competition, or other activity (“Activity”) of the Grand Rapids Women’s Golf Association is voluntary. You hereby acknowledge that there are risks associated with attending or participating in the Activity. Further, you, for yourself and on behalf of your heirs, assigns, personal representatives, next of kin, and guests hereby release and hold harmless the Grand Rapids Women’s Golf Association (DBA GRWGA) and it’s officers, directors, owners, officials, volunteers, employees, agents, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the Activity with respect to any and all injury, illness, disability, death, or loss or damage to person or property whether arising from the negligence or releases or otherwise, to the fullest extent permitted by law.

As a result of the highly contagious novel coronavirus, COVID-19, federal, state and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.

GRWGA cannot guarantee that you or your heirs, assigns, personal representatives, or next of kin will not become infected with COVID-19. Further, attending and participating in any GRWGA Activity could increase you and your heirs, assigns, personal representatives, or next of kin’s risk of contracting COVID-19.

By signing up for membership and GRWGA events I acknowledge and agree to the following:

  • I will keep myself informed of and abide by any and all directives designed to lower the risk of COVID-19 exposure which may include wearing masks, social distancing (6 feet apart), washing hands, non-sharing of equipment, wiping down all shared equipment after each use, and other directives as determined by the governing bodies or health agencies of the State of Michigan, any and all local governing bodies or health agencies and any and all golf courses, restaurants, bars, and anywhere else where the Activity may occur from time to time.

  • If I fail to follow these directives, I will forfeit my right to continued participation in the Activity and any other membership rights I may have in GRWGA. This forfeiture of rights will continue whether or not GRWGA or its officers, agents, volunteers or other representatives observe or call out any failure of your following these directives at any time and further, the forfeiture of these rights and discontinuation of participation is your obligation to notice, acknowledge, and self-impose at all times.

  • In no way is the GRWGA, its officers', volunteers, employees, agents or other representatives obligated to inform you or monitor your compliance with said directives as they may exist and change from time to time.

I attest that I will not participate in or attend Activity if I:

  • Am experiencing any symptom of illness including but not limited to, a fever, cough, or shortness of breath.

  • Have been exposed to anyone with a confirmed or suspected case of COVID-19 within 14 days of Activity

  • Have travelled internationally within 14 days of Activity

If I have been diagnosed with COVID-19, I will not participate or attend Activity unless I have been medically cleared to do so.

BY SIGNING UP FOR MEMBERSHIP AND PARTICIPATING IN AN EVENT WITH THE GRWGA I AM CONSENTING TO THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, AND FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY AGREEING TO IT AND FULLY ACCEPT THIS FOR MYSELF AND MY HEIRS, ASSIGNS, GUESTS, PERSONAL REPRESENTATIVES OR NEXT OF KIN AND AGREE TO IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
