Assumption of Risk and Waiver of Liability

Your participation and/or attendance at any event, competition, or other activity (“Activity”) of the Grand Rapids Women’s Golf Association is voluntary. You hereby acknowledge that there are risks associated with attending or participating in the Activity. Further, you, for yourself and on behalf of your heirs, assigns, personal representatives, next of kin, and guests hereby release and hold harmless the Grand Rapids Women’s Golf Association (DBA GRWGA) and it’s officers, directors, owners, officials, volunteers, employees, agents, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the Activity with respect to any and all injury, illness, disability, death, or loss or damage to person or property whether arising from the negligence or releases or otherwise, to the fullest extent permitted by law.

BY SIGNING UP FOR MEMBERSHIP AND PARTICIPATING IN AN EVENT WITH THE GRWGA I AM CONSENTING TO THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, AND FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY AGREEING TO IT AND FULLY ACCEPT THIS FOR MYSELF AND MY HEIRS, ASSIGNS, GUESTS, PERSONAL REPRESENTATIVES OR NEXT OF KIN AND AGREE TO IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
